There is an increased risk of virus infection with SARS-CoV-2. Please respect prescribed measures (published on GOV.SI Portal https://www.gov.si/en/topics/coronavirus-disease-covid-19/, and relevant UL web pages) and follow UL FFA guidelines that determine eligibility to be present at the FFA premises.

Class attendance registration

When entering the laboratories at the FFA for each practical you will register with the help of QR code and then register your UL digital identity on the website (the first time you register, it is saved on your phone and so the next time you enter the UL digital identity entry is no longer required). The data will be stored in the server register for 30 days: username, lecture room or laboratory and date and time of registration. After 30 days, the data will be deleted automatically. If you do not have a smartphone at hand, registration is possible on the lecturers’ computer in the lecture room or at the receptionist for practical work. This will allow us to monitor your presence and facilitate the organization of the study process in times of exceptional circumstances due to SARS-CoV-2.

To scan a QR code, you need a device (mobile phone, tablet) and a suitable program for reading the QR code. In most new phones, such a program is already part of the operating system. If the program is not installed, we suggest installing the program:

Registration

By registering, I declare that:

I meet one of the PCT's prescribed conditions and can demonstrate this to an authorised person at all times when I am present on UL FFA premises;
I enter the Faculty premises in good health and do not have any of the following symptoms/signs: fever, cough, headache, malaise, sore throat, runny nose, shortness of breath, diarrhoea;
I will consistently and correctly wear a clean mask on the Faculty premises, observe cough hygiene and hand washing and disinfection;
I will fully comply with and respect all instructions given by UL and UL FFA.

In order to speed up the reporting of identified high-risk close contacts (HRCs) to the National Institute of Public Health (NIJZ), please fill in the following criteria. I am:

I have complied with the exemption from quarantine (I have received a booster dose of Comirnaty vaccine from Biontech/Pfizer or Spikevax vaccine (COVID-19 Vaccine) from Moderna)

I do not meet the exception to quarantine

I don't want to answer

If you complete the declaration, it will be forwarded to the NIJZ for the purpose of notifying identified high-risk close contacts (HRCs), in accordance with the Guidance, but only in the event of an infection occurring in your study group.

Data handling

The Faculty of Pharmacy, Aškerčeva cesta 7, Ljubljana, processes your personal data for the purposes of organising study activities and tracking high-risk close contacts in the event of SARS-CoV-2 infection in a particular group. We process your personal data on the basis of Article 6(1)c and Article 9(i) of the GDPR, in conjunction with the regulations in force from time to time. The types of data that will be collected and stored in the electronic record on the basis of the registration are:

location (lecture theatre, laboratory) of the registration;
date and time of registration;
student ID (digital identity);
information on the fulfilment of the quarantine exemption (CP or T).

Data retention will be limited to up to 30 days, after which the data will be automatically and irreversibly destroyed.

In the event of a detected infection in a particular group, we will use your registration and the registration of the remaining students to track or identify high-risk close contacts, whose data will be shared with the NIJZ, based on the applicable regulations.
